Financial Worries Add to Cancer Patients' Burden

WEDNESDAY, June 6 (HealthDay News) -- A small study gives a snapshot into the financial anxieties that plague many patients with advanced cancer and their spouses, even as they struggle against the disease itself.

Four of every five such American patients and their spouses-caregivers in the study said they had concerns about meeting medical costs and suffered "financial stress." Worries about paying medical costs also were tied to lower mental and physical health, the study found.

"Across the board, the longer they were in treatment or reaching the end of life, there were [financial] concerns. There were concerns whether it came to their own well-being or their families' well-being," said study lead author Fay Hlubocky, a clinical psychologist and ethicist at the University of Chicago Pritzker School of Medicine.

She reported the findings this week at the annual meeting of the American Society of Clinical Oncology (ASCO), in Chicago.

The study involved 52 patients with advanced cancers, all of whom were enrolled in clinical trials, and their spouses. While patients in clinical trials are a slightly different group than that seen in the general population (because some expenses of treatment may be paid for), Hlubocky said the numbers from her study "are borne out in the literature generally" for patients battling cancer outside of such trials.

Patients ranged in age from 28 to 78, with a median age of 61. All were married, two-thirds had more than a high school education, and just over half made less than $65,000 a year. Forty-five percent of the patients were employed, as were two-thirds of spouses-caregivers. Each patient and their spouse were asked about a wide range of financial concerns, and they also took part in standard tests assessing depression, anxiety, quality of life and mental/physical health.

The researchers found that at the beginning of the study, 82 percent of the patients and 69 percent of the spouses reported "medical cost concerns," while 79 percent of the patients and 81 percent of spouses said they had "financial stress." Queried a month later, the level of "medical cost concerns" had risen -- 85 percent of patients and 72 percent of spouses now cited such worries.

People who encountered "unexpected" costs related to care had higher anxiety and depression scores than those who did not, the study found. Quality-of-life scores were lower for patients who had financial worries, and the physical and mental health of the spouses-caregivers seemed to decline as medical care cost worries persisted, the study found.

What were patients and their spouses worried about? According to Hlubocky, it ranged from the "little things" -- parking and hotel accommodations, gas and mileage getting to and from doctors' appointments -- to much larger concerns, including insurance coverage (or lack thereof), how to provide for loved ones after death, and even bankruptcy.

Some participants expressed real anxiety in meeting their financial obligations. "We had to pay for an additional hospitalization for a small-bowel obstruction, and insurance would not cover it," one patient told the researchers. "If we had to sell our house to pay, we'd do it."

Other patients felt their illness threatened their livelihood. "My employer has an attendance policy that if violated too many times will result in termination," the patient said. "My appointments have to be midday usually." The patient considered going on disability, "but that would not pay for my insurance."

One expert said these types of fears are all too common for people coping with cancer.

"It is certainly true that the impacts beyond diagnosis and treatment are tremendous for cancer patients," said Dr. Sylvia Adams, ASCO spokeswoman and assistant professor in the department of medicine at NYU Langone Medical Center, in New York City. "They do face several challenges. And as this article shows, there is a substantial number of patients who feel that there is anxiety and depression and lower quality of life associated with worries about financial stability."

Adams said that, even for people with insurance, costs can quickly escalate. These include medication co-pays, transportation costs, time missed from work, child-care issues and the cost of in-home medical devices.

However, both Hlubocky and Adams stressed that, as challenging as things can be for cancer patents, help and resources are out there. First and foremost, they said, is to make sure you have a social worker on your cancer-care team who can point you in the right direction for help.

"There's lots of different foundations and coalitions out there that are willing to help," Hlubocky said. "Certain hospitals, you can just go and talk to the social worker and try and find out what's the best way to help cover some of the financial problems. Many patients don't know. A social worker is an absolutely wonderful person to have on your team."

Adams agreed. "It is very important that the treating team has a multidisciplinary aspect, and that it involves psychological support as well," she said, since the stress of dealing with cancer and its treatment can be overwhelming.

Findings presented at medical meetings are typically considered preliminary until published in a peer-reviewed journal.

Why antipsychotics need time to kick in

Insight into how drugs work may explain the delayed action of medications for schizophrenia

New details about how some drugs for schizophrenia accumulate in the brain may help explain why patients often must wait for weeks for the medications to work.

Because many commonly used antipsychotics such as haloperidol and clozapine quickly latch onto their targets, it would seem that the drugs should bring fast relief.

?But there?s always a side story,? says neuroscientist Michael Cousin of the University of Edinburgh in Scotland. ?There?s another layer of complexity.?

Researchers led by Teja Groemer of Friedrich-Alexander University of Erlangen-N?rnberg in Germany illuminate this process in the June 7 Neuron by describing how the buildup of certain drugs in the brain may have underappreciated consequences for their effectiveness.

The idea that drugs accumulate in the brain isn?t totally new; other scientists have suggested that antipsychotic drugs can pile up in certain places, Groemer says. But most people have assumed such accumulation is inconsequential.

Not so, Groemer and his team found. Stockpiled drugs may actually squelch nerve cells? behavior in a highly selective way by being released only when needed most.

Using a proxy compound that could be seen with a microscope (because making the drugs themselves visible would have changed their behavior), the researchers watched the chemical build up in small pockets, called synaptic vesicles, inside nerve cells that were growing in a dish. Normally, these vesicles? cargo is cell-manufactured molecular messages. In an excited nerve cell, vesicles rise to the surface, glob on to the cell?s outer membrane and float their messages out and away.

Vesicles chock full of unnatural cargo ? haloperidol, chlorpromazine, clozapine and risperidone ? also dump their contents into the space between nerve cells when a cell is excited, the team found. This evacuation sends a ?calm down? signal back to the cell that releases the drug. In this way, the activity of a nerve cell dictates the drugs? effects. The more active a cell, the more antipsychotic drug it gets. ?The brain doses itself,? Groemer says.

This accumulation process might help explain why people typically have to take such drugs for several weeks before noticeably improving: The drugs need time to accumulate in the vesicles before exerting their calming effect. Other drugs with similar chemical makeups, including some antidepressants, may behave similarly.

The experiments were conducted with cells in dishes and with rodents; it?s not clear whether the same thing happens in people. If so, the results could have important implications for doctors, Groemer says. ?One really needs to wait for a drug to accumulate before deciding whether the drug is effective or not.?

Also equally important yet gets overlooked all too often: Get a solid budget in place, and make sure that you have enough room in your budget to allow for anything unexpected that may come up. If you?re about to undertake a major remodeling project for instance, you could easily find yourself dipping into your pockets to cover additional wiring or plumbing, for instance. The general rule of thumb is at least 20% of your estimated budget should be available to cover any cost overruns.

It?s a tried-and-true expression: Bathrooms and kitchens sell houses. A remodeled kitchen and/or bathroom can often return a good portion of your investment when you sell your house. However, don?t make the mistake of focusing all your efforts on one or two rooms while leaving the rest of your home untouched.

Stay conservative with any improvements. While that new swimming pool might sound tempting to you, it?s not likely to be a big selling point. Ideally, any changes you make to your home should be things that can be changed relatively easily such as new paint. Your goal should be not to make so many improvements to your home that you literally price it out of the neighborhood.

While it?s not an expense you should undertake lightly, a room addition can be another excellent way to raise the value of your home. An extra bedroom in particular is a great selling point for many homebuyers. Having your home inspected by an appraiser or real estate agent prior to constructing an addition can give you a reasonably good idea of how it?s likely to affect the value of your home.

According to Doll, putting such needs and wants onto paper turns a relationship into something ?nerdtastic and not particularly romantic,? as if the only things that people ever fight over are inconsequential issues like chores and takeout preferences. ?

In a perfect world, of course each and every couple would truthfully lay their neuroses and peccadilloes on the line from the beginning in a tough but enlightening conversation, and the partners would choose carefully whether they wanted to proceed.     From the Muncie Star-Press: The city-county plan commission gave its approval to $2.6 million in public financing on Thursday to help finance a $19.2 million expansion at Spartech Packaging Technologies on the south side. The 60,000-square-foot expansion at the Spartech food packaging manufacturing facility bordered by Memorial Drive, Macedonia Avenue, 18th Street and Penn Street, will add 32 jobs. Current employment is 114 and the current space is 167,000 square feet.     The Basics

    Artificial sweeteners can curb cravings. Low-calorie sweeteners, aka sugar substitutes, are used to sweeten foods and drinks and at the same time reduce calories. Artificial sweeteners do not contain carbohydrates so they do not raise blood sugar. That said, many foods and beverages that are sweetened artificially still contain calories from carbohydrates, which also causes blood glucose to rise. Be sure to read labels carefully so you can keep a close eye on overall carb intake.

    Which Artificial Sweetener is the Healthiest

    There are five artificial sweeteners with U.S. Food and Drug Administration approval. They are found in foods and beverages, and also sold packaged to use in cooking and baking. The five artificial sweeteners available today are:

    • acesulfame potassium (Sunett, Sweet One)
    • aspartame (Nutrasweet, Equal)

    How Artificial Sweeteners Can Help

    Small changes can make a big difference. Swapping artificially sweetened beverages for sugar-sweetened ones can save you dozens of grams of carbohydrates and hundreds of calories, every day. For example, if you drink two 12-ounce sodas daily, you?re consuming 80 grams of carbohydrates and 300 calories. If you choose diet soda, or zero-calorie sweetened soda water, you?re saving a ton of calories. If you swap 2 teaspoons of sugar in your morning coffee for one packet of artificial sweetener, you save 32 calories and 10 grams of carbohydrates.

    Artificial Sweetener Side Effects

    Although artificial sweeteners have been under the microscope of the public eye for decades, there is no clear evidence that artificial sweeteners cause any side effects. Studies in the early 1970s linked saccharin to increased rates of bladder cancer in laboratory rats, but subsequent studies proved conclusively that these results applied only to rats, and not to humans.

    There was a scare in the mid-1990s when a report was released suggesting that aspartame might have led to an increase in the number of people with brain tumors between 1975 and 1992. However, a closer look at the data shows no proof whatsoever to these claims.

    Numerous studies in the past few decades have confirmed that artificial sweeteners are safe in reasonable quantities, even for pregnant women.

    6 Tips That Can Help Affair-Proof Your Relationships


    The term cheating is one that elicits cringes of fear, gasps of horror.

    What comes to mind when you hear it?

    Most likely you imagine that a partner in a committed relationship had sexual intercourse with someone outside of their relationship.? But cheating can look like many things to many people.

    To some it may indeed refer to sexual intercourse only.? To others it could be anything from an emotional attachment to another, fantasies of other partners, a kiss.

    Rather than use the term cheating here, something that makes me think more of copying someone else?s answers on a test and less about who you share your body or heart with and when, I am going to use the term infidelity.

    I define infidelity as any action that violates an implicit or explicit agreement between two people, thereby undermining the relationship.? The action may be physical or emotional in nature.? Dishonesty is often but certainly not always part of an infidelity.

    As a relationship and sex therapist, infidelity is not a new or foreign topic to me.? It is in fact, one of the most common causes of couples seeking counseling.

    Infidelity, whether considered, presumed, or committed.? To most couples, infidelity signifies a crisis.

    Some can repair the damage done, turn an infidelity into an opportunity for growth and reconnection.? And some can?t, the loss of trust being irreparable for one, the continued anger and blame intolerable for the other.

    I tend to be pretty proactive and pre-emptive in my work so my focus here is to talk about what I believe leads to infidelity and what I think you can do about it.

    But first, a disclaimer. I am an LPC (licensed counselor) with a private practice in Portland, OR.? Portland is a very sex positive city with a visible and strong female presence.

    The majority of my clients are ages 20-40 and most of them, particularly the women, I have noticed, have done a lot of thinking and talking about their sexuality before coming to see me.

    What that means is that I am working with a pretty savvy group of folks here and they have a good grasp on what they want out of their sex lives.

    Among this group of men and women, there is a fairly equal mix of those for whom a committed and monogamous long-term relationship is a goal and those for whom it is not.

    More and more individuals and couples I work with are trying out open relationships because the concept of sexual exclusivity doesn?t jive with their worldview.

    Some of the couples I work with who sought counseling due to an infidelity did so because they were trying to maintain a monogamous relationship based on their partners needs (or ultimatums) but couldn?t.

    Certainly though, it is just as common for monogamy to have been a clearly stated (and not only assumed) mutually agreed upon desire and choice.

    But the risk in monogamous relationships is that sex can start to feel lacking or uninteresting over time.? Some describe feeling like they or their partners feel like sex is more of a chore then an enjoyable way to get or give pleasure or connect.

    I have many times heard it stated in session that they aren?t ready to give up and resign themselves to a sexless life and that if their partner can?t meet their needs, they will get them met elsewhere.

    So what leads to affairs??

    Here is what I see.? Reasons that are either 1. Emotional 2. Physical or 3. Practical

    1. Emotional ????

    Probably the most commonly cited cause of infidelity is a sense of emotional disconnection from a partner.? The person engaging in the infidelity will often describe having felt unappreciated, lonely, and sad.? They might fear abandonment or rejection.

    These emotions can often lead to feelings of anger and resentment.? Emotional dissatisfaction can lead to affairs both emotional and physical in nature.

    People may seek out others for emotional support and validation.? Or sex may be withheld from a partner because of anger.

    2. Physical ????

    In some cases a partner who has engaged in an infidelity will cite sexual dissatisfaction as being the reason they strayed.

    In these cases sex may be not as often as they would like or as fulfilling as they would like; either because they are not receiving pleasure or reaching climax or because it lacks a certain chemistry or passion.

    While for some, physical relationships can only feel enjoyable or safe when there is trust and intimacy, for others those same factors can make sex feel strange or awkward.

    Some pairings are also just not as sexually compatible as others.? What turns one person on might make another deeply uncomfortable.

    3. Practical ????

    One thing I have observed in my practice is that there has been a shift in thinking, especially in younger generations, about the practicality and benefits of monogamy.

    More and more people are choosing lifestyles and relationships that are non-monogamous.? But there is not yet a road map for how to have successful open relationships so people are crossing boundaries without a clear plan, honest communication, and clear expectations.

    For instance, the primary factors that delineate an open relationship from an infidelity are mutual agreement and honesty.?

    Here are the 6 things I believe everyone can do in order to minimize the risk of infidelity.?

    1. Do the work it takes to get to know yourself as a sexual person.

    Read books, take a workshop, talk to a counselor, journal.? Study your sexuality as you would any other subject you were trying to master.

    Increase your own awareness about what you like and don?t like, what you fantasize about, how you like to be touched and where, what you would like to try.

    2. Communicate who you are sexually to your partner

    Don?t assume that they know!? Have an honest conversation at a time when you both feel relaxed.? This conversation doesn?t have to be about your particular sexual relationship with your partner, though that can be part of it.

    It?s really about you.? Share as much as you filling willing to about what you learned from your work mentioned above.

    3.? Ask questions and listen to who they are as a sexual person.

    Learning about your partner?s desires and fantasies can both increase intimacy and be terribly fun!? You might think you know what your partner likes but chances are, unless you have asked them, you don?t know all of it.

    One of the best things that can come from open dialog about sexuality is that it reminds partners of their own autonomy.? In order to desire someone else, they simply must be acknowledged as being a separate individual than you!

    Knowing that you are curious about them as a sexually autonomous individual interested in exploring with them and can also help reduce risk that your partner will stray.

    4.? Keep an open mind and heart and be willing to try new things together.

    Try not to judge. In the realm of sexuality, almost anything goes (I say almost because the one caveat is that if another person is involved, it must be consensual).

    People?s sexual desires and expressions are hugely varied and you should avoid making assumptions about what they mean as it is unique to each individual.

    A general rule of thumb is to try anything once.? Getting outside of your comfort zone sexually can be an amazing growth experience and you may find that you are deeply attracted to things you had not before considered.

    5. Affairs are distractions and people distract themselves when they are bored or unhappy.? An affair is a ?quick fix? with the long-term consequence, even if not discovered.

    If you are bored or unhappy in your relationship, it?s easy to be more ?open? to others.? Something that may start as a lighthearted flirtation or pleasant conversation can quickly deepen and become a consuming distraction.

    This is a slippery slope and can ultimately undermine any hopes of improving things with your partner.? Instead of distracting yourself and avoiding the problem, try to remain present and engaged with your partner.

    Check yourself to see if you are doing something that you are not telling your partner about and ask yourself if it would make them uncomfortable if they did.? You and your partner share mutual responsibility for your relationship.

    6.? Be honest.? Honest with yourself and honest with your partner.

    Too many couples avoid the topic of sex, especially when it has been a long period of time since partners have had sex, because the subject feels too tense and overwhelming.

    Continued avoidance makes it harder and harder to bridge the gap.? Honesty early on (before an affair) about your concerns can create an opening for a new level of intimacy.

    Honesty after an affair will demonstrate accountability and remorse.? Many partners who ?discover? an affair say that the discovery and the deception were the worst part.

    I strongly believe that any couple can improve their sex life at any time.? The biggest hurdle is starting to talk about a problem that has been ignored, like an elephant in the room.

    Because of this, I highly recommend that couples call out the issue quickly and with respect and compassion.? Seeking the help of a counselor can facilitate this process and help break down any tension that might be building, providing a safe space in which to talk about it.

    Couples can also read books or watch movies together (yes, this can include pornography) in order to open up a conversation about sexual experiences that you or your partner might be interested in having.? It is much easier to prevent an affair from happening then to repair the effects of having one.

    Yes, your sexual needs are important.? If, after following the steps above, you find that you and your partner are still not able to enjoy sex together or remain committed to an agreement of monogamy, then the relationship might not be right for you.

    If that is the case, discuss it with your partner and see if you want to work together to create a new plan or can agree to part ways with understanding and kindness.
